Job Description

We are seeking a compassionate, experienced, and motivated Director of Social Services to join our dedicated interdisciplinary team at a skilled nursing and rehabilitation center. The ideal candidate will advocate for residents' psychosocial well-being while ensuring compliance with all federal and state regulations. Responsibilities Develop and oversee the facility's social services program Conduct psychosocial assessments and care planning Assist residents and families with adjustment, counseling, and discharge planning Facilitate and/or supervise programming, treatment, and discharge of resident's placement and release as assigned. Complete timely and accurate documentation in accordance with agency policies and procedures, contracts, and regulatory requirements and standards. Frequently transverse from one area of the facility to another, including interior and exterior areas. Meet predetermined deadlines and reporting requirements for the position. Coordinate community resources and support services Participate in interdisciplinary care team meetings Ensure compliance with CMS, state, and facility regulations Address resident rights, grievances, and family concerns Maintain accurate documentation and records
Critical Action Items & Measurable Deliverables:
Meet all federal and state regulatory guidelines and standards that are applicable to this position. Develop proactive plans of action for every area of service assigned by the Program Director or Associate Executive Director/Director. Provide timely feedback and support to personnel. Review all staff documentation to ensure it is accurate, complete, and timely. Consult, provide feedback, review performance, motivate, direct, and redirect subordinates. Adapt schedules to the needs of the program based on changing workloads and needs. Organize and manage time and resources to ensure timely completion of all assignment deadlines. Monitor facility video to ensure compliance with action plans, contractual, and regulatory requirements. Participate in scheduled meetings and in team decisions and operations. Work evenings, weekends, and holidays as needed or requested by the position supervisor. Maintain confidentiality in all areas of the service population and program operations. Qualifications Bachelor's Degree in Social Work, Human Services, or related field required Licensed Social Worker preferred Minimum 2 years of skilled nursing facility or long-term care experience Strong knowledge of Medicare, Medicaid, and discharge planning Excellent communication, organizational, and leadership skills Compassionate and resident-centered approach
Requirements:
Pass a pre-employment drug screen and random drug screens throughout employment. Pass a pre-employment and biennial criminal background checks.
Demonstrate the ability to:
A. Respond sensitively and competently to the service population's cultural and socio-economic characteristics.

B. Work collaboratively with other staff members, service providers, and professionals.

C. Work in a fast-paced environment and maintain emotional control and professional composure at all times.

D. Maintain computer literacy required to meet the responsibilities of the position.
