QIDP/Case Manager

Primary Function:

To assure quality care and training are provided to persons served in the implementation and delivery of Active Treatment Services on a 24- hour basis.

Responsible To:

Director of Active Treatment Performance Requirements:

Must be able to read, write, type, make calculations and to follow oral and written directions. Must be able to communicate effectively both orally and in writing.

Physical Demands and Working Conditions:

Lifting, pulling, and pushing required. Moderate data entry. Work may be performed inside or outside of assigned group home. Mobility from one place to another is required.

Qualification:

Experience with active treatment necessary. A Bachelor's Degree in a related field and at least one-year experience in the field of intellectual disabilities is required. A valid Tennessee driver's license with clear MVR is required. Supervisory experience is preferred.
