Schedule:

M-F 8:30am-5pm

Location:

Brandon, FL. Role is fully onsite. What you'll Do Enter scheduling data into Homecare Homebase HCHB and schedules clinical staff according to care plans/evaluations, location, and staff availability. Process all incoming patient referrals from physicians and acute/non-acute facilities. Verify Medicare benefit eligibility through on-line DDE checks; and if necessary, confirm beneficiary's status with other agencies by phone. Determine and communicate medical complexity/acuity of all referred patients and their orders to the Clinical Coordinator. Coordinate patient care with various members of the healthcare team, including physicians, physician's office staff, hospital discharge planners/case managers, and Territory Managers. EMR experience with Homecare Homebase (HCHB) is ideal. High School Diploma or GED is required. Currently licensed as a Licensed Practical Nurse (LPN) in Florida is preferred. Demonstrates strengths in collaboration and partnership to attain strategic goals, organization, problem-solving, time management, decision-making, and follow-through. Advanced interpersonal skills including verbal and written communication skills. Able to work autonomously with minimum supervision.
