We are currently seeking an experienced, organized, and proactive Home Health Scheduler to join our Clearwater office. This is a critical role that serves as the coordination hub between our patients, clinicians, and office staff, ensuring timely and efficient delivery of home health services. Position Summary The Home Health Scheduler is responsible for coordinating and managing patient visits for clinical staff while ensuring continuity of care, staff productivity, and compliance with agency requirements. The ideal candidate is highly organized, detail-oriented, and able to effectively manage multiple priorities while maintaining a positive and professional demeanor. Essential Responsibilities Schedule and coordinate patient visits for RNs, LPNs, HHAs, Physical Therapists, Occupational Therapists, Speech Therapists, and Medical Social Workers. Optimize clinician schedules to maximize productivity and ensure timely patient care. Communicate schedule updates, cancellations, and changes to staff and patients promptly. Monitor visit frequencies and ensure visits are scheduled according to physician orders and care plans. Maintain accurate scheduling records and data entry within the agency's EMR system. Collaborate closely with Clinical Managers, Intake Coordinators, and field staff to address scheduling needs. Assist with staffing coverage and schedule adjustments due to call-offs, patient requests, or changes in care needs. Generate reports and provide scheduling support to management as needed. Ensure compliance with agency policies, Medicare guidelines, and regulatory requirements. Provide exceptional customer service when communicating with patients, caregivers, referral sources, and staff. Qualifications High School Diploma or GED required. Minimum of one (1) year scheduling experience in a healthcare setting preferred. Home Health experience strongly preferred. Experience with EMR systems and healthcare scheduling software preferred. Strong organizational, time management, and problem-solving skills. Excellent verbal and written communication abilities. Ability to multitask and prioritize effectively in a fast-paced environment. Proficient in Microsoft Office and computer-based systems. Ability to work independently and as part of a collaborative team. Why Join Classic Home Health Services?
