Job Description
We are seeking a compassionate, organized, and motivated Care Coordinator to support youth residing in a therapeutic group home environment. The Care Coordinator plays a critical role in ensuring that residents receive individualized care, emotional support, and access to services that promote stability, growth, and independence. This position works closely with youth, families, case managers, therapists, schools, and community providers to coordinate care plans and ensure a safe, structured, and supportive living environment. Responsibilities Coordinate and monitor individualized treatment and service plans for youth residents Maintain regular communication with families, guardians, social workers, therapists, schools, and external providers Advocate for youth needs and ensure access to educational, medical, behavioral health, and community resources Conduct intake assessments and assist with transition planning and discharge coordination Maintain accurate case documentation, progress notes, incident reports, and compliance records Support youth in developing independent living, social, and coping skills Participate in team meetings, case reviews, and crisis intervention as needed Ensure compliance with state licensing standards, agency policies, and confidentiality requirements Assist with transportation coordination and appointment scheduling when necessary Foster a positive, trauma-informed, culturally responsive environment for all residents Experience Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or related field preferred Minimum of 1-2 years of experience working with at-risk youth, behavioral health, foster care, residential treatment, or group home settings Strong communication, organization, and crisis management skills Knowledge of trauma-informed care and youth development practices Ability to work collaboratively with multidisciplinary teams Valid driver's license and acceptable driving record CPR/First Aid certification preferred (or willingness to obtain) Preferred Skills Experience with case management software and electronic documentation Familiarity with Medicaid, DSS/CPS systems, or behavioral health coordination Ability to de-escalate crisis situations professionally and calmly Bilingual applicants encouraged to apply This role is ideal for motivated individuals passionate about delivering compassionate care while supporting efficient healthcare operations.
