Job Description
Behavioral Health Network (BHN) is the largest provider of behavioral health services in Western Massachusetts and a Certified Great Place to Work. If you are passionate about supporting children, youth, and families, and have strong skills in care coordination and collaboration, we invite you to join our Intensive Care Coordination (ICC)/CSA team as a Care Coordinator II. In this role, you'll facilitate the Wraparound process for youth ages 0-21 with behavioral/emotional needs, helping them and their families access critical supports and services.
Conduct comprehensive assessments, including
Convene and facilitate Care Planning Team (CPT) meetings to develop and update Individualized Care Plans (ICP).Maintain consistent communication with youth, families, and care teams to identify strengths, needs, and effective strategies.
Support youth and family participation in goal-setting and implementation.
Identify and connect families with medically necessary services.
Collaborate with state agencies, schools, healthcare providers, and community resources.
Develop transition plans when goals are met.
Obtain insurance authorizations as required by Managed Care Entities.
Maintain service productivity and caseload expectations.
Ensure timely documentation and completion of Risk Management/Safety Plans.
Familiarity with internet, word processing, and email systems.
Experience navigating child/family-serving systems (preferred).Knowledge of community resources (preferred).Experience facilitating meetings or groups (preferred).Demonstrated cultural competence and ability to work with diverse populations.

What You'll Do:

(12+), diagnostic assessments, and Serious Emotional Disturbance determinations.Convene and facilitate Care Planning Team (CPT) meetings to develop and update Individualized Care Plans (ICP).Maintain consistent communication with youth, families, and care teams to identify strengths, needs, and effective strategies.
Support youth and family participation in goal-setting and implementation.
Identify and connect families with medically necessary services.

Master's Degree in a Human Services-related field (license-eligible).CANS-MA certification (required).Knowledge of mental illness/SED in children/adolescents and treatment techniques.Familiarity with internet, word processing, and email systems.
