WeCare Centers is actively hiring a Care Transitions Coordinator for our skilled nursing facility in Murrysville,PA Make an Impact Beyond the Bedside In this role, you'll be a key leader in ensuring our residents experience safe, coordinated, and successful transitions of care—from admission through discharge. If you have experience in skilled nursing and are passionate about patient safety, care coordination, and improving outcomes, this is a rewarding opportunity to make a real difference. What You'll Do Oversee the resident journey from admission through discharge, ensuring safe and smooth transitions Coordinate with nursing, therapy, social services, and physicians to support effective care planning Monitor interdisciplinary team (IDT) performance related to safety, care management, and discharge readiness Identify and address potential risks, including falls, pressure injuries, and avoidable readmissions Develop and implement strategies to improve patient safety and discharge outcomes Conduct audits and assessments to ensure compliance with regulations and best practices Maintain accurate documentation related to care plans, safety initiatives, and discharge processes Provide education and guidance to staff on safety protocols and transition planning What We're Looking For Experience in a Skilled Nursing Facility (SNF) required Background as an RN, LPN, Social Worker, Therapist, Case Manager, or Nursing Home Administrator (NHA) Strong understanding of care transitions, discharge planning, and regulatory standards Excellent communication and collaboration skills Ability to manage multiple priorities in a fast-paced environment Passion for improving resident outcomes and quality of care Why WeCare Centers Competitive pay based on experience Same Day Pay option Health, Dental, and Vision Insurance Voluntary Benefits Employee Assistance Program (EAP) 401(k) Growth opportunities across a multi-facility organization About WeCare Centers At WeCare Centers, we believe in not only providing exceptional care for our residents but also ensuring a supportive and enriching environment for our team.
