Job Description
Position Summary The Caregiver is responsible for providing direct care, support, supervision, and assistance to residents in accordance with individualized service plans, facility policies, state regulations, and resident rights. The Caregiver promotes resident independence, dignity, safety, and quality of life while assisting with activities of daily living (ADLs), monitoring resident conditions, and maintaining a positive living environment. Essential Duties and ResponsibilitiesResident Care Assist residents with Activities of Daily Living (ADLs), including: Bathing Dressing Grooming Toileting and continence care Mobility and transfers Eating and hydration support Provide cueing, standby assistance, hands-on assistance, or total care based on resident needs and service plans. Assist residents with ambulation using gait belts, walkers, wheelchairs, and other approved equipment. Support residents in maintaining dignity, independence, and choice whenever possible. Observe and report changes in resident condition, behavior, appetite, mobility, cognition, skin integrity, or mood. Resident Safety and Monitoring Conduct resident safety checks and routine rounds. Respond promptly to call lights, emergency pendants, alarms, and resident requests. Monitor for fall risks, wandering behaviors, environmental hazards, and changes in functioning. Follow fall prevention, elopement prevention, infection control, and emergency response procedures. Immediately report incidents, injuries, accidents, medication concerns, or unusual occurrences to nursing staff or supervisors. Documentation Document care provided accurately and timely in the facility documentation system. Record ADL assistance, observations, behaviors, intake/output when required, and resident concerns. Complete required shift reports and communicate pertinent information during handoff. Resident Engagement Encourage participation in activities, socialization, and community programs. Promote resident choice, independence, and meaningful engagement. Provide emotional support and companionship to residents and families. Housekeeping and Environmental Support Maintain clean and safe resident environments. Assist with bed making, light housekeeping tasks, laundry support, and room organization as assigned. Observe rooms for maintenance, cleanliness, or safety concerns and report appropriately. Infection Prevention and Control Follow infection prevention practices and PPE requirements. Adhere to hand hygiene standards. Follow isolation precautions and outbreak procedures as directed. Assist in maintaining sanitary conditions throughout the community. Team Collaboration Work collaboratively with caregivers, Med Techs, nurses, RCC, dietary, activities, housekeeping, maintenance, and administration. Attend staff meetings, in-services, and required training. Support teamwork and positive workplace culture. Qualifications High school diploma or GED preferred. Previous caregiving, healthcare, assisted living, memory care, or long-term care experience preferred. Current caregiver training or certifications as required by state regulations. CPR/First Aid certification preferred or obtained within required timeframe. Ability to complete required state orientation and ongoing education. Knowledge, Skills, and Abilities Knowledge of resident rights and confidentiality requirements. Understanding of ADLs and resident-centered care. Ability to communicate effectively with residents, families, and staff. Ability to recognize and report changes in condition. Ability to maintain professionalism and confidentiality. Strong customer service and teamwork skills. Physical Requirements Employee must be able to: Lift, push, pull, and transfer residents as permitted by policy. Frequently stand, walk, bend, reach, kneel, and assist with transfers. Move throughout the facility for extended periods. Respond quickly during emergencies. Use approved lifting and transfer equipment. Working Conditions Exposure to bodily fluids, infectious diseases, and cleaning chemicals. Potential exposure to challenging behaviors, including residents with dementia or behavioral health needs. Weekend, evening, holiday, and on-call work may be required.
