Job Description
Position Summary The Registered Nurse (RN) is responsible for providing clinical oversight, nursing assessments, resident care coordination, staff support, and regulatory compliance within the Assisted Living community. The RN supports resident health and safety through assessment, monitoring, care planning, staff education, delegation, and coordination with providers, families, pharmacies, hospice agencies, and outside healthcare partners. The RN assists in ensuring services are provided according to resident service plans, physician orders, state regulations, and facility policies while promoting resident independence, dignity, and quality of life. Essential Duties and ResponsibilitiesClinical Assessment and Resident Oversight Complete nursing assessments, evaluations, and clinical reviews as required. Assess resident physical, cognitive, behavioral, psychosocial, and functional status. Monitor residents for changes in condition and initiate appropriate interventions. Participate in admission assessments, re-assessments, and service planning processes. Assist with determining appropriateness for admission and ongoing residency. Review hospital, SNF, hospice, and provider documentation for care coordination. Conduct skin assessments, fall reviews, behavior reviews, and wellness monitoring. Participate in resident risk assessments and mitigation planning. Service Plans and Care Coordination Assist in development, review, and revision of individualized service plans. Ensure services align with resident needs and physician recommendations. Participate in interdisciplinary care conferences.
Coordinate services with:
Primary care providers Specialists Hospice agencies Home health Therapy providers Behavioral health teams Pharmacies Monitor implementation of interventions and care outcomes. Medication and Treatment Oversight Review medication orders and treatment instructions for accuracy. Monitor medication administration processes and resident responses. Provide nursing guidance regarding medication concerns, adverse reactions, and changes in condition. Oversee clinical treatment programs within RN scope. Assist with narcotic discrepancy investigations and medication audits. Ensure medication systems comply with facility procedures and applicable regulations. Delegation and Staff Support Delegate nursing tasks when permitted under state regulations. Train, supervise, and support caregivers and medication technicians. Conduct competency validations and clinical observations. Provide coaching regarding resident care standards and documentation. Assist with orientation and onboarding of new clinical staff. Participate in performance improvement activities. Resident Safety and Regulatory Compliance Monitor resident incidents, falls, injuries, behaviors, and hospital transfers. Conduct investigations and follow-up for clinical events. Assist with abuse prevention, mandatory reporting, and resident rights compliance. Participate in survey preparation and corrective action planning. Support infection prevention and outbreak management activities. Ensure compliance with Assisted Living regulations, facility policies, and nursing standards. Documentation Responsibilities Complete nursing documentation accurately and timely. Maintain clinical records and resident assessment information. Review caregiver documentation for accuracy and completeness. Document change of condition reviews and interventions. Participate in quality assurance audits. Infection Prevention and Control Monitor infection trends and symptoms within the community. Implement isolation precautions and PPE procedures when necessary. Provide staff education related to infection prevention. Assist with outbreak response planning. Monitor compliance with hand hygiene and sanitation procedures. Family and Provider Communication Communicate resident status updates to families and responsible parties as appropriate. Coordinate with providers regarding changes in condition. Participate in family meetings and care conferences. Assist families with understanding resident care needs and available services. Leadership Responsibilities Provide clinical leadership and support to caregivers and Med Techs. Promote teamwork and accountability. Assist RCC and administration with clinical operations. Participate in quality improvement projects. Support staffing coverage and workflow coordination when needed. Qualifications Current unrestricted Registered Nurse (RN) license in the State of Oregon. Graduate of an accredited nursing program. Current CPR certification. Experience in Assisted Living, Memory Care, SNF, home health, hospice, or geriatrics preferred. Supervisory or leadership experience preferred. Knowledge, Skills, and Abilities Knowledge of Assisted Living regulations and resident rights. Understanding of geriatric nursing and chronic disease management. Knowledge of dementia care and behavioral interventions. Ability to perform clinical assessments and identify changes in condition. Strong leadership and communication skills. Ability to train and mentor staff. Proficiency with electronic medical records and documentation systems. Physical Requirements Employee must be able to: Stand and walk for extended periods. Assist with resident mobility and transfers when necessary. Lift, push, pull, bend, stoop, and reach frequently. Respond rapidly to emergencies. Travel throughout the community multiple times per shift. Working Conditions Exposure to infectious diseases, bodily fluids, and cleaning chemicals. Interaction with residents experiencing dementia, behavioral symptoms, or complex medical conditions. Potential weekend, holiday, after-hours, and on-call responsibilities. Regulatory Responsibilities (Oregon Assisted Living Focus) The RN shall assist in ensuring compliance with: Oregon Assisted Living regulations and resident service requirements Resident assessment and change-of-condition procedures Abuse reporting requirements Infection control standards Medication oversight requirements Service planning expectations Resident rights protections Documentation and quality assurance requirements Performance Expectations The RN will be evaluated on: Resident outcomes and safety Service plan accuracy and timeliness Reduction in incidents and hospitalizations Staff competency and education completion Documentation compliance Regulatory survey performance Family satisfaction Clinical quality indicatorsPay:
