Trinity Support Group Services Position Summary:
At Trinity Support Group, our caregivers are the foundation of the support we provide. The Caregiver / Direct Care Worker (DCW) is responsible for delivering compassionate, person-centered care to individuals in a home and community-based setting. This role supports individuals with daily living tasks, encourages independence, and ensures a safe, respectful, and structured environment. Provides non-medical personal care and supportive services to clients in their homes, in accordance with the approved service plan, to promote safety, dignity and independence. Duties and Responsibilities Caregivers may perform the following as authorized in the client's service plan and/ or by the caseworker: Provide direct care support with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and mobility Assist with mobility, transfers, and ambulation Assist with meal preparation, feeding, and maintaining proper nutrition Assist with housekeeping and laundry Prepare meals and assist with feeding if needed Complete shopping and errands Provide companionship and emotional support Implement individualized service plans (ISP) and behavior support strategies Monitor and document client progress, behaviors, and any significant changes Support individuals in community-based activities such as appointments, outings, and social engagement Maintain a clean, safe, and organized home environment Utilize appropriate de-escalation techniques to ensure client and staff safety Complete required documentation including daily logs and incident reports in a timely manner Communicate effectively with supervisors, team members, and families
Qualifications:
High school diploma or GED required Previous caregiving, direct support, or behavioral health experience preferred Experience working with individuals with developmental disabilities and/or mental health diagnoses is a plus Valid driver's license and reliable transportation Ability to pass background check and drug screening CPR/First Aid certification (or willingness to obtain upon hire)

Work Environment & Physical Requirements:
Ability to assist with lifting, transferring, and supporting individuals (up to 50 lbs) Frequent standing, walking, bending, and physical assistance Flexible availability, including evenings, weekends, and holidays as needed Work performed in residential and community settings Must be able to assist clients with movement, transfers and daily activities.
Service Limitations:
Only perform tasks approved in the service plan/caseworker authorization No skilled nursing services No Medication administration No medical procedures Starting Pay & Compensation •
Starting Pay:
$17.50 per hour (based on experience and qualifications) • Eligible caregivers will also gain access to our 401(k) Retirement Plan, with flexible contribution options per pay period •
Growth Opportunities:
Pay increases, bonuses, and advancement opportunities are available through our Retention & Growth Incentive Program 90-Day Probationary Raise Caregivers who successfully complete their first 90 days with dependable attendance and strong performance will receive a $0.50/hour raise •
Paid Training:
$150 stipend for completing required online training and shadowing (13 total hours), issued after two weeks of active service •
Sign-On Bonus:
$200 awarded after successful completion of 90 days of employment o Must maintain good standing o No call-offs during the 90-day period
