Plan Referral Program Responsibilities:

Provide in-home care and support to clients Assist with personal care and hygiene support (bathing, grooming, toileting). Assist with mobility and transfers, ensuring client safety. Administer medication reminders and assist with management. Foster emotional well-being through companionship and social activities. Prepare nutritious meals based on dietary needs. Maintain a clean-living environment with light housekeeping. Provide transportation and/or running errands as needed. Document care and report changes in client conditions.

Requirements:

Education:

High school diploma or equivalent required.

Experience:

Preferred experience with young adults, adults, elderly, or disabled individuals.

Skills:

Excellent communication and interpersonal abilities.

Transportation:

Reliable vehicle and valid driver's license required.

Background Check:

Must pass a criminal background check and drug screening.

Flexibility:

Willingness to work nights and weekends is a plus.

Physical Requirements:

Ability to lift up to 50 pounds. Comfortable with bending, standing, and navigating stairs.
