Day shift | Part-time | No weekends or holidaysSalary:
$33.51 - $49.00 per hour How you'll make an impact in this role Coordinate and oversee the interdisciplinary plan of care from admission through discharge. Develop and monitor continuum-of-care plans that incorporate patient and family preferences. Advocate for appropriate resources, remove barriers, and collaborate with supervisors and care teams to ensure effective plan implementation. Serve as a resource for clinical documentation accuracy, service appropriateness, and post‑discharge care planning. Charting is done on a tablet using Point Care to be done in the home. What minimum qualifications you'll need Licensure / Certification /Registration:
One or more of the following required: Clinical Counselor obtained prior to hire date or job transfer date. Counselor obtained prior to hire date or job transfer date. Social Worker obtained prior to hire date or job transfer date. Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Case Manager credentialed from the American Case Management Association (ACMA) preferred.Education:
Master's degree in Social Work required.Work Experience:
