Case Management Nurse

Overview Are you a Registered Nurse looking to use your skills to review cases and find the right care for your patients? Join our Home Health team serving rural and tribal communities and gain valuable hands-on experience while making a real difference in patients' lives. We are seeking compassionate and motivated RNs interested in developing their skills in home health case management . You will receive guidance and support while providing patient-centered care in the home environment. Responsibilities Perform patient assessments in the home setting Assist with developing and implementing care plans Provide basic skilled nursing care and patient education Coordinate with physicians and care teams Qualifications RN License Strong communication and organizational skills Must pass national and multi-jurisdictional background checks Valid driver's license TB and drug screening tests Interest in community-based and rural healthcare

Pay:

$40.00 - $50.00 per hour
