GENERAL DUTIES AND RESPONSIBILITIES
  • To maintain confidentiality in all situations.
  • To ensure that annual paperwork is completed (rights, grievance, money, transportation, Hippa, etc.)
  • To enter into support note system all telephone conversations, person to person conversations, medical visits, or any other pertinent information.
  • To function and work with others as a member of a team advocating for individuals' needs.
  • To assist with maintaining the database with Service Recipient's most current data and dates.
  • To be flexible in handling work assignments to accommodate Service Recipients, staffs, planning meetings, etc. keeping the County Administratorinformed of schedule.
  • To assist with Internal Quality Assurance as assigned.
  • To assist in teaching and encouraging Service Recipients to be as independent as possible.
  • To attend any meetings as scheduled.
  • To attend weekly Incident Review Committee meetings as assigned
  • To perform other duties as assigned.
SPECIFIC DUTIES AND RESPONSIBILITIES
(may be divided between other staff as assigned)
  • To be responsible for for Service Recipients supported.
  • To maintain proper procedures in distributing all prescription medication.
  • To maintain a close relationship with the pharmacy
  • To provide the MAR to residences on a monthly basis.
  • To check the MAR on a monthly basis as determined by Quality Assurance.
  • To be responsible for filling and distributing all prescription medications.
  • To ensure that non-prescription medications are authorized by physician.
  • To administer medications in the day program when needed.
  • To schedules, take, and provide documentation and follow up for all Service Recipients medical appointments.
  • To maintain annual physical and dental exams for all Service Recipients.
  • To arrange and monitor all assessments (e.g. medical, psychological, social, leisure, education, etc.)
  • To report results of health assessments and discuss pertinent findings with individual, relevant staffs, parents, families, Independent Support Coordinators and to document all discussions in database.
  • To prepare paperwork for, and meet with psychiatrist on a quarterly basis. To maintain Service Recipient's files
  • keeping them current, purging items over two years old, keeping overflow file up-to-date.
  • To maintain database for each Service Recipient including dates, changes, ISP, etc.
  • To assist with medical visits as needed.
  • To assist with Individual Support Plans for Service Recipients with an Independent Support Coordinator.
  • To make "home visits" once per month using the Quality Assurance checklist as assigned.
  • To complete discharge summary of any individual leaving a program within 15 days of discharge.
  • To maintain contact with Service Recipient's Planning Team, ISC and family.
  • To support individuals receiving services through the
ECF/CHOICES
program.

  • A Bachelor's degree in Social Work, Special Education, or a closely related field or a Licensed Practical Nurse .
  • Good driving record; current Class D license with F endorsement.
  • Knowledge of computer usage is essential.
  • The Case Manager shall be self-directed and capable of working closely with other staff members.
