  • Full-time Overview This position is responsible for assisting clients with case management services that support engagement in treatment, continuity of care, and long-term recovery from substance use and co-occurring mental health disorders.
Responsibilities include coordinating ancillary services, facilitating communication between clients and treatment providers, assisting with discharge and aftercare planning, supporting client advocacy efforts, and ensuring timely and accurate documentation. The Case Manager works collaboratively with the multidisciplinary treatment team to identify and address barriers that may impact a client's ability to participate successfully in treatment. This includes assistance with medical, vocational, educational, financial, legal, housing, family, and community resource needs. This position requires professionalism, organization, strong communication skills, sound judgment, and the ability to maintain therapeutic boundaries while supporting a recovery-oriented treatment environment. Tasks and Responsibilities Admissions and Client Coordination Responsibilities Assists with client admissions, intakes, and orientation processes as assigned. Completes non-clinical intake documentation and required administrative assessments. Reviews agency rules, client rights, confidentiality policies, and program expectations with clients. Conducts pre-admission and case management interviews to identify client needs and potential barriers to treatment. Obtains appropriate releases of information and coordinates communication with referral sources, employers, family members, and outside providers as authorized. Assists clients with employment, school, legal, disability, FMLA, and related documentation needs. Communicates admission and transition updates to the treatment team as appropriate. Case Management Services Responsibilities Responsible for coordinating non-clinical support services and assisting clients in accessing resources necessary to support treatment participation and recovery. Meets with clients regularly to assess ongoing case management needs and treatment barriers. Assists clients with accessing community resources including medical, psychiatric, legal, educational, vocational, financial, and housing services. Coordinates referrals and communication with outside agencies and providers as appropriate. Assists clients in obtaining required documentation, identification, insurance information, and other necessary records. Supports continuity of care and assists with transition planning throughout treatment. Advocates for client needs while maintaining professional boundaries and ethical standards. Maintains awareness of client progress and communicates significant concerns to the treatment team. Discharge and Aftercare Planning Responsibilities Responsible for assisting in the development and coordination of discharge plans and recovery support services to promote continued stabilization and recovery following treatment. Collaborates with clients and the multidisciplinary treatment team to develop individualized aftercare and discharge plans. Coordinates referrals to outpatient treatment providers, recovery residences, medical providers, support groups, and community resources. Ensures discharge documentation and transfer records are completed accurately and timely. Assists in coordinating communication with referral sources and continuing care providers. Encourages client participation in recovery-oriented support systems and follow-up care recommendations. Client Support and Professional Conduct Responsibilities Responsible for maintaining professional interactions with clients while supporting a safe, respectful, and recovery-oriented treatment environment. Interacts with clients in a professional, respectful, and supportive manner. Maintains therapeutic and professional boundaries at all times. Assists in reinforcing program expectations, rules, and behavioral standards. Utilizes sound judgment when responding to client concerns, behavioral issues, or crisis situations. Reports emergencies, safety concerns, or significant client issues to supervisory staff immediately. Supports a positive treatment environment through teamwork, professionalism, and effective communication. Assists with group facilitation and recovery support activities as assigned and within scope of training. Internal Communication and Collaboration Responsibilities Responsible for maintaining effective communication with treatment team members and supporting coordinated client care. Participates in multidisciplinary treatment team meetings and staffing. Communicates relevant client updates and concerns to clinical and operational staff. Collaborates with counselors, medical staff, behavioral health technicians, and leadership to support client care. Assists in supervising and supporting Behavioral Health Technicians as assigned. Maintains effective working relationships with internal departments and external providers. Documentation and Administrative Responsibilities Responsibilities Responsible for maintaining accurate, timely, and organized documentation in accordance with agency policy and regulatory standards. Completes all required documentation accurately and within established timelines. Maintains documentation within the electronic medical record system in accordance with agency standards. Protects client confidentiality in compliance with
HIPAA, 42 CFR
Part 2, and agency policies. Ensures documentation reflects services provided and client participation. Completes reports, correspondence, and administrative tasks as assigned. Maintains organized and efficient work areas and assists with operational workflow as needed. Professional Responsibility Responsibilities Responsible for adhering to professional, ethical, and organizational standards while supporting ongoing professional growth and development. Adheres to all applicable federal, state, and agency regulations and policies. Maintains confidentiality and ethical conduct at all times. Demonstrates professionalism, accountability, reliability, and teamwork. Participates in required trainings, supervision, and continuing education activities. Maintains awareness of current behavioral health and substance use treatment practices. Performs additional duties as assigned within scope of position responsibilities.
