Case Manager

SUMMARY OF FUNCTION

Provides strengths based case management services for families on campus. Functions as a member of the Case Management Team and other teams as assigned. May be assigned to work the Helpline/Coordinated Assistance Database. May serve in an on-call after hours capacity to support campus residents.

RESPONSIBILITIES

Reports directly to the Program Director. Responsible to function as a member of the Case Management Team. Responsible for maintaining the confidentiality of designated information. Responsible for the day-to-day provision of case management services. Responsible for providing services in accordance with agency and Program goals. Responsible for the reporting of suspected child abuse or neglect, as a Mandated Reporter as defined by Act #238 of the Public Acts of 1975.
