CASE MANAGER
Tri-County Juvenile Hearing Board Program Part-time Location:
Johnston Position Summary Tri-County Workspace is seeking a dedicated and compassionate part-time Case Manager to support the Juvenile Hearing Board Program serving the towns of Smithfield and North Providence, RI. The Case Manager will work directly with youth and families referred to the Hearing Board, providing assessment, coordination, and follow-up services aimed at supporting positive youth outcomes and reducing further involvement in the juvenile justice system. Key Responsibilities Conduct intake assessments and screenings for youth referred to the Juvenile Hearing Board Work collaboratively with youth and their families to identify needs, strengths, and appropriate supports Develop individualized service plans and provide referrals to community-based resources Monitor and document participant progress and compliance with Hearing Board recommendations and/or sanctions Maintain regular follow-up with youth, families, and service providers Collaborate with Hearing Board members, schools, law enforcement, and community partners Maintain accurate, timely, and confidential case records Prepare reports and provide updates as required Prepare quarterly data and assist with grant reporting Qualifications Bachelor's degree in Social Work, Human Services, Psychology, Criminal Justice, or a related field (or equivalent experience) Experience working with youth and families, preferably in a community-based or juvenile justice setting Strong interpersonal, communication, and organizational skills Ability to work independently and as part of a team Knowledge of local community resources is a plus Requirements Must pass a background check Valid driver's license and reliable transportation preferred Must have evening availability to attend
