M-F 8-5Pay Range:
$24 to 27 per hour (DOE/DOQ)Hours:
Full-time, 40 hours per week Job Summary Provides on-site and community-based case management services to adults, supporting recovery goals outlined in Individual Treatment Plans. Services are delivered in a person-centered, strengths-based, and culturally responsive manner. Key Responsibilities Coordinate and deliver case management services to meet clients' behavioral health and basic needs Develop and maintain recovery-oriented, person-centered service plans Conduct ongoing assessments of client strengths and needs Assist clients with housing, benefits, transportation, education, and vocational goals Teach life skills, including problem-solving, symptom management, and use of community resources Advocate for clients and coordinate care with hospitals, providers, and community agencies Provide supportive counseling and psychoeducation (individual, family, and group) Maintain timely, accurate documentation and ensure confidentiality compliance Participate in team meetings, trainings, and program developmentQualifications Required:
Bachelor's degree in Behavioral Science or related field Two years of case management or clinical services experience Eligible for Agency Affiliated Counselor registration (assistance provided) Valid driver's license and insurable driving record Ability to pass background check, drug screening, and provide required immunizationsPreferred:
