Full-time General Description:
Mary's Haven is a 15 month residential program for pregnant women and their children.
POSITION SUMMARY
The Case Manager is a full-time position that provides Case Management support to each resident at Mary's Haven. The Case Manager will assess each new resident's Case Management needs, develops the resident's individualized Case Plan, implements the Case Plan and documents the participant's progress throughout their stay in the program. The Case Manager will also manage staff scheduling as we are staffed 24/7, monitor and inventory items needed for proper running and maintenance of the home. The Case Manager will stay in compliance with established programming and rules related to both staff and residents.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Intake calls Communicate program policies and expectations to intake in a clear and detailed manner and confirm understanding. Complete drug screens for new resident. Assist with completion of intake paperwork. Gather documentation from each resident. Assist resident in obtaining needed social services, education, employment, and mainstream benefits for which he/she is eligible. Collect, document, and report program fee and escrow deposits. Writing Incident Reports for serious incidents as well as program violations. Provide daily case management to residents. Develop personalized case plans for each resident and update regularly as goals/needs are met or changed. Monitor residents progress on goals of Case Plan, documenting progress and updating goals and action steps as appropriate to maintain Case Plan effectiveness and relevancy. Overseeing congregate activities, such as life skill classes. Coordinating community service hours and other activities. Create and maintain client files in a confidential matter. Enter residents' confidential data in HMIS. Operate company vehicles to manage donations, transport clients, and any other tasks as directed. The Case Manager is provided with a cell phone specifically for coworkers, supervisors, residents and intakes to make contact when needed, and must be answered at all times during business hours. The Case Manager may be asked to attend workshops, seminars, conferences or in-house training relating to her position and responsibilities. This may occasionally require overnight travel. Prepare weekly reports for Program Director. Additional tasks as requested by Program Director or Executive Director. The Case Manager may be asked to perform other tasks and duties not specifically outlined in this job description, but within the scope of his/her education, experience, and abilities, as necessary for the successful operation of the agency.
QUALIFICATIONS
Ability to listen to, understand, and speak so others can understand ideas and information presented verbally. Must be able to work as a team member and get along well with fellow employees. Proven ability to work under the supervision of management. Demonstrated ability to adapt and function in a diverse environment. Must be understanding, patient and empathetic to the demographics of the women we serve. Ability to work in stressful environment and with people of various backgrounds. Must be professionally assertive at times and enforce program guidelines and infractions. Ability to work flexible hours when warranted. Must be able to respond appropriately in a crisis situation. Must possess thorough knowledge of all policies and regulations relevant to working with women in crisis. Must possess thorough knowledge of community resources and state assistance programs
EXPERIENCE
Proven ability to work under the supervision of management. A minimum of 2 years of case management experience. Proven ability to work with high level of detail. Demonstrated ability to adapt and function in a diverse environment.
