Schedule varies from 8:00 a.m. to 7:00 p.m. Monday•Friday Varying work hours including evening and weekends may be required

OF DUTIES AND RESPONSIBILITIES
Overview:
The Case Manager is responsible for implementing and documenting social services related to the client's individualized treatment plan objectives. Social Services performed by the Case Manager include those activities that improve and maintain the client's functioning in the home and community. The Case Manager is responsible for carrying a caseload, teaching treatment-related classes, scheduling mental health appointments, and completing intake and discharge plans. The Case Manager also performs other related duties as required.
Duties:
The duties listed below are intended to illustrate work functions performed by the position. I.
Client/Program Duties:
1. Maintains a positive atmosphere conducive to agency philosophy and humane treatment conditions. 2. The Case Manager works with the Deputy Director Director to prepare a record of client needs and coordinates the appropriate referrals for the clients, makes appropriate follow-up on all arranged referrals, including mental health, individual counseling, and documents all activities in accordance with established procedures. 3. Tracks client's treatment plan needs and assists the client in working toward completion of the Individualized Treatment Plans. Provides input with regard to developing Individualized Treatment Plan and Discharge Plan. 4. Prepares applicable client program passes. 5. Meets regularly with each client on caseload. 6. May act as a representative for the Case Manager Department or agency at outside meetings, committees or functions. 7. On a continuous basis, update community resources for all four counties. 8. Assists clients in securing appropriate and affordable post-program living arrangements within the approval of the assigned Probation Department personnel. 9. Documents contacts with probation officers, attorneys and other officials relative to a client's participation in the program. 10. Must be ORAS certified to administer and score the
ORAS. 11.
Facilitates and documents assigned structured groups. 12. Group facilitation must adhere to group lesson plans. Deviation from lesson plan it not permitted. Documents class progress notes at the end of each class. 13. Arranges for, or provides resources to help meet client's needs including the transportation for established appointments to obtain necessary documents, etc. 14. Prepares written reports as required. 15. Serves as Team leader for weekly Treatment Team meetings. Acts as Team Coach for assigned clients. II.
General Duties:
1. Understands and adheres to policy and procedural requirements of
SRCCC. 2.
Maintains accurate records. Must be computer literate. 3. Maintains strict confidentiality with regard to any and all information obtained and processed in the operation of the program. 4. Utilizes audio visual equipment on an as-needed basis. Maintains general knowledge of equipment and its use. Notifies managing information specialist of equipment malfunction or tampering. Notifies maintenance department of any other equipment or furnishing repair need. 5. Meets with Program Director on a continuous basis to share information regarding client programming progress. Participates in scheduled All-Staff Meetings and other meetings as required. 6. Works with the Program Director to continuously upgrade the services provided through the Case Manager Department. 7. Assists in maintaining the client case files as per agency policy. 8. Supplements facility coverage. 9. Be energy conscious and stress conservation. 10. Completes a minimum of forty (40) hours of in-service training per year, including 24 hours of evidence-based practices. Participates in seminars and continuing education programs when appropriate and feasible. 11. Subject to being called into work at any time due to emergency situations and/or coverage needs. 12. May be required to provide transportation assistance to clients, SRCCC employees, and documents.
Minimum Qualifications:
Education:
Bachelor's degree in social work, counseling, psychology, criminal justice or other related behavioral science field preferred; ORAS Certified, and CDCA Certificate preferred or the ability to obtain within six months of hire, Associate's level in criminal justice or related field required plus one-year experience.
Experience:
One-year experience required for Associate's level, two years preferred. Working knowledge of criminal justice system, behavioral contracting process and individual and group interactive processes preferred.
Skills:
Must be computer literate. Ability to work professionally with co-workers and supervisors. Ability to establish and maintain effective working relationships with clients, co-workers, outside agencies, etc. Ability to gather, collate and classify information about data, people or things. Ability to work under stressful and demanding conditions. Ability to effectively manage time and organize workload. Skill in oral and written communication. Ability to work independently. Ability to facilitate group work and courses.

Automobile Liability Insurance and Valid Ohio Driver's License is required .
