Case Manager

Position Summary We are a national behavioral health provider with an inpatient location in VA. We are looking for a part-time weekend Case Manager . The Case Manager plays a critical role in supporting clients throughout their treatment journey in a structured detox and residential setting. This position is responsible for coordinating care, assisting with discharge planning, connecting clients to community resources, and ensuring continuity of services in alignment with clinical treatment plans and regulatory standards. The ideal candidate has prior experience working in a clinical or case management capacity within a behavioral health or substance use treatment center , understands the continuum of care, and can effectively advocate for clients while collaborating with interdisciplinary teams.

Schedule:

Full Time 8:30a-5p Monday-Friday w/ some rotating weekends Key Responsibilities Provide comprehensive case management services for clients in detox and residential levels of care Collaborate with clinical, medical, and admissions teams to support individualized treatment planning Conduct psychosocial assessments, resource evaluations, and service coordination as assigned Facilitate discharge planning, including aftercare placement, housing coordination, and referral scheduling Connect clients with community resources such as outpatient providers, MAT programs, therapists, support groups, and social services Maintain regular communication with families, referral sources, and external providers (as authorized) Assist clients with benefits coordination, transportation arrangements, and appointment scheduling Monitor client progress and address barriers to treatment engagement or step-down placement Complete timely, accurate documentation in accordance with program policies, state regulations, and accreditation standards (Joint Commission/CARF as applicable) Participate in multidisciplinary team meetings, case conferences, and utilization reviews as needed Support crisis intervention and client advocacy efforts when appropriate

Qualifications Required:

CSAC, CSAC-S, or CSAC-T required Prior experience in a clinical, counseling support, or case management role within a behavioral health or substance use treatment center Knowledge of substance use disorders, recovery models, and levels of care (detox, residential, PHP, IOP, OP) Strong organizational, communication, and documentation skills Ability to manage multiple cases in a fast-paced inpatient environment

Preferred:

Experience with discharge planning and community resource coordination Familiarity with EMR systems and insurance/UR processes Experience working with co-occurring mental health and substance use disorders

Pay:

$45,000.00 - $52,000.00 per year
