Job Description
Job Overview We are seeking a compassionate and dedicated Certified Home Health Aide to join our team in providing exceptional in-home care to individuals with diverse needs. As a vital member of our caregiving staff, you will deliver personalized support that promotes independence, safety, and well-being. This role offers the opportunity to work closely with clients in their homes, delivering essential health services and emotional support while fostering a respectful and empowering environment. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using Hoyer lifts or other assistive devices. Monitor patient observations by tracking vital signs such as blood pressure, pulse, temperature, and respiration; report any abnormal findings promptly. Administer medications and manage medication records in compliance with HIPAA regulations and medical documentation standards. Support clients with specialized care needs such as Alzheimer's disease, dementia, memory care, hospice care, and working with individuals with developmental disabilities or disabilities. Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems accurately to document patient care activities and medical records. Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients. Use medical terminology appropriately during documentation and communication with healthcare teams; assist with catheter care and other medical procedures as trained. Employ patient observation skills to recognize changes in condition or behavior; communicate effectively with family members and healthcare providers. Utilize equipment such as Hoyer lifts safely; assist with transfers and mobility to prevent falls or injuries. Provide emotional support and companionship to promote mental well-being and foster trustful relationships. Experience Prior experience in home health care or assisted living environments is highly preferred. Demonstrated ability to work with seniors or individuals with disabilities in long-term care settings or nursing homes. Knowledge of medical terminology, medication administration protocols, and vital signs monitoring. Experience working with individuals who have Alzheimer's disease or dementia-related conditions is advantageous. Familiarity with EMR/EHR systems for accurate medical documentation is required. Certification in CPR (Cardiopulmonary Resuscitation) is mandatory; additional certifications such as hospice care training or catheter care are a plus. Proven ability to handle sensitive situations with compassion while maintaining professionalism within HIPAA guidelines.
