Clackamas, OR and surrounding communities within a 60-mile radius (Clackamas/Portland-Beaverton/Tigard, occasionally Salem/McMinnville, if needed.)

We're seeking a Hospice Certified Nursing Assistant (CNA) to serve patients in the Clackamas, OR and surrounding areas. You'll work closely with our nursing and interdisciplinary team while providing essential comfort-focused care in patient homes and facilities. About the Role As a Hospice CNA , you'll provide personal care and supportive services under the direction of the RN Case Manager and interdisciplinary team. This Role If You:
Bring empathy, patience, and kindness to every visit Communicate clearly with patients, families, and the care team Are comfortable working independently in the field Take pride in providing respectful, hands-on personal care Understand the importance of reliability and consistency in hospice care Find meaning in supporting patients at end of life This position is ideal for CNAs who value compassion, teamwork, and meaningful work . Key Responsibilities Provide personal care including bathing, grooming, toileting, and mobility assistance Assist with safe transfers and positioning Observe and report changes in patient condition to the RN Case Manager Support patient comfort and dignity at end of life Provide emotional support to patients and families Document care provided according to hospice standards Follow infection control and safety protocols You'll visit 5-6 patients daily depending on travel distance and care complexity.
