The ideal candidate will provide essential support to patients in our skilled nursing home setting. This role involves assisting with daily living activities, monitoring patient health, and ensuring a safe and comfortable environment for all patients. Responsibilities Assist patients with daily activities such as bathing, dressing, grooming, and eating. Monitor and record vital signs and report any changes in patient condition to nursing staff. Provide basic care to patients including catheter care, medication administration, and assistance with mobility using equipment such as Hoyer lifts. Maintain accurate medical documentation and ensure compliance with HIPAA regulations. Support patients with memory care needs, including those with Alzheimer's or dementia. Collaborate with healthcare teams to develop and implement patient care plans. Ensure cleanliness and safety of patient areas while adhering to aseptic techniques. Assist in emergency situations by providing basic life support when necessary. Skills Candidates should possess a variety of skills to excel in this role, including: Experience with EMR/EHR systems such PCC. Basic knowledge of medical terminology, anatomy, and physiology. Experience in long-term care settings such as nursing homes or assisted living facilities. Proficiency in vital signs monitoring. Strong communication skills for patient service and teamwork within a healthcare setting.
