Responsibilities Deliver compassionate patient care by assisting with activities such as bathing, dressing, feeding, and mobility using tools like Hoyer lifts and other assistive devices Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature using EMR (Electronic Medical Records) systems such as Epic or Cerner Support medication administration under supervision while adhering to HIPAA regulations and aseptic techniques to ensure patient safety Assist with specialized care needs including dementia care, Alzheimer's care, memory care, and behavioral health support Prepare patients for procedures or examinations in outpatient clinics or hospital settings such as ICU or Level I Trauma Centers Maintain cleanliness and infection control standards through proper use of PPE (Personal Protective Equipment) and adherence to infection prevention protocols Document all patient interactions thoroughly in medical records ensuring compliance with medical terminology standards and legal requirements Skills Proven experience working in diverse healthcare environments including long-term care facilities, nursing homes, assisted living communities, hospitals (including ICU and trauma centers), or home care settings Familiarity with EMR/EHR systems like Epic, Cerner, eClinicalWorks or similar platforms for accurate documentation Strong knowledge of physiology, anatomy, medical terminology, and aseptic techniques necessary for safe patient handling and care delivery Ability to perform phlebotomy procedures and basic life support (BLS) certification preferred but not required Experience caring for individuals with disabilities or developmental delays; expertise in dementia or memory care is highly valued Skilled in using assistive devices such as Hoyer lifts and working with patients requiring complex mobility assistance Excellent communication skills to collaborate effectively with multidisciplinary teams across outpatient clinics, inpatient units, or community-based programs Join our team to be part of a vibrant environment where your caregiving skills are celebrated!
