Position:
Certified Peer specialist (CPS) (Assertive Community Treatment)
Location :
Langhorne, PA Department:
Assertive Community Treatment The ACT Certified Peer Specialist provides Wellness Recovery Action Planning (WRAP), Illness Management and Recovery (IMR) as well as individual and group support to the ACT Team. The Certified Peer Specialist operates as the lived experience expert of the team and assists with crafting engagement and treatment planning for clients who could benefit from CPS supports. Under the direct supervision of the Team Leader, the Peer Specialist is responsible for sharing his or her personal, practical experience, knowledge, and insight to benefit the team and its clients.
Qualifications:
High school diploma or GED, Pennsylvania Peer Specialist Certification, valid driver's license required. WRAP facilitator training and Peer Support Whole Health facilitator training strongly preferred.
Essential Duties:
Provide peer counseling and support to validate clients' experiences and provide guidance and encouragement Serve as a mentor to clients to promote hope and empowerment Collaborate with other team members to promote culture in which client experiences are recognized, understood and respected Assist clients in applying for income and health related benefits such as SSI and SSDI Assist clients in the coordination of transportation to necessary medical, housing, and social services appointments Connect individuals to additional services and resources as needed, including food, clothing, health care, employment and training etc. Provides linkage with and accompanies consumers to community resources and social opportunities, as defined in their individualized service plansI Increase level of community integration Utilizes strengths-based approach and recovery-oriented models of treatment Conduct individual & group support/education sessions Maintain consumer focus on personal recovery and strengths •Assist in the development of WRAP's with consumers Complete required documentation and paperwork, in a timely manner, including service recording and service planning in the EHR Regularly attend and participate in daily organization staff meeting Provide ongoing assessment of D&A and Mental Health issues Completes Treatment and Safety Planning paperwork for their primary clients. Conduct Illness Management and Recovery sessions with group clients. Provides direct primary over-sight of paperwork and case management support for 8-12 people. Follow policies and procedures of the agency in administrative, clinical, quality improvement, and human service areas Maintain good daily and required meeting attendance Maintain timely documentation of all work assignments and unusual incidents for risk management Routinely keep supervisor informed about attendance and job assignments Work cooperatively with peers and participates in all required CQI activities
Physical Requirements:
This position normally requires the physical demands of standing, walking, bending, lifting, sitting on the floor or performing other work requiring low physical exertion, ability to move quickly in a crisis situation, talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time.
