Job Description
Job Summary The Certified Enrollment Coordinator is primarily responsible for assisting patients by conducting enrollment and verification activities for assistance programs such as Healthy Families, Medi-Cal, and related health insurance programs in a FQHC setting. This position also includes providing CHW and ECM services to clients in the community.
FQHC Setting Duties:
(may not include all duties assigned) 1. Conduct new patient screenings before the patient's office visit. 2. Assist new patients in our orientation process, including new patient registration forms, health care coverage, other resource eligibility screening, site orientation materials, and care team information. 3. Assists patients with application processes for health care coverage and other services, including CalFresh, DMV placards, or other programs/services. 4. Advises and assists patients with application and follow-up to determine eligibility for appropriate income-eligible programs, including sliding scale, CDP, EWC, FPact, etc. 5. Stay current with changes within patient assistance programs and services through bulletins, workshops, and websites. 6. Responsible for ensuring all applicants are enrolled promptly and compliant with all applicable rules and regulations, including follow-up on necessary documents. 7. Responsible for responding to phone calls from applicants and performing administrative functions as required. 8. Responsible for attending and participating in scheduled staff meetings and planned meetings as required, including appropriate Certified Enrollment Counselor meetings. 9. Answer phone calls, routed calls, and scheduled patient appointments. 10. Manage information entry into the electronic health record, such as patient demographics, faxes, previous medical records, and radiology results, and assist with communication with other medical facilities through the electronic health record. 11. Reviews sliding scale applications for accuracy. Accurately accounts for fees for service for patients using financial assistance programs. 12. Assist with clinical data outcome reporting and actively participate in quality improvement projects. 13. ADAP enrollment for all qualified patients, depending on site assignment. 14. Other duties as assigned.Summary of Programs:

a.Care Management Services:
Oversee care management services, including assessments, care plans, interventions, goal expectations, and coordination, to ensure holistic care. b.Communication and Collaboration:
Ensure effective communication with CHWs, healthcare providers, Partnership Health Plan, and community resources, facilitating seamless transitions and care coordination. c.Client Progress:
Monitor and evaluate program outcomes, identifying areas for improvement and implementing best practices. CHW Integration & Transitions of Care Services (review of CHW services is required)CHW Services:
a.Collaboration and Communication:
Collaborate with community organizations to ensure seamless transitions are achieved, and care coordination is developed, focusing on the unique short-term needs of clients and developing relationships with Hospitals and community organizations. b.Care Planning and Coordination:
Ensure effective communication and care planning addresses the unique needs of the client post-hospitalization to prevent readmissions and promote preventative care and health equity. c.Transition Planning:
Facilitate smooth transitions to community-based care, addressing social determinants of health and ensuring continuity of care through f/u PCP appointments and community resources.NON-ESSENTIAL DUTIES AND RESPONSIBILITIES
Performs other duties as assigned. Ability to work independently and be self-directed and flexible. Ability to perform functions with minimal supervision. Ability to work at a high-volume level of accuracy.Minimum Requirements:
1. Certified Application Assister strongly preferred. 2. Previous experience in related community work is preferred 3. Experience in an outpatient setting or working with a health clinic is desirable. 4. Experience with the Medi-Cal application process and program, as well as other assistance programs, is preferred. 5. Bilingual (Spanish) strongly desired 6. Knowledge and experience of medical front office duties. 7. Computer literacy and familiarity with fax, copier, and calculator machines. 8. Maintaining good relations with the public and the Health Center staff while working under pressure.Expectations:

Expected to have strong interpersonal skills and the ability to understand and follow written/verbal instructions. 9. Able to work independently with little or no supervision. 10. Required to be able to deal effectively with diverse individuals at all organizational levels. 11. Carry a caseload of 12-18 clients.Requirements:
1. Community-based work or in-vivo work is required with proper safety precautions. 2. Certification as a CHW (CCHW) or willingness to obtain within 18 months. 3. CPI and CPR within 60 days of hire. 4. Strong communication, interpersonal, and cultural competency skills. 5. Ability to work independently and as part of a team. 6. Basic computer skills and familiarity with electronic health records. 7. Valid driver's license and reliable transportation (if applicable).Preferred Qualifications:
1. A bachelor's degree in psychology, social work, human services, or a related field. 2. Certified as a CHW (CCHW) 3. Experience working with diverse populations and communities. 4. Knowledge of community resources and services. 5. Familiarity with care management software and technology.Working Conditions:
1. FQHC clinic, office, and community-based setting with travel to client homes, community organizations, county office, and healthcare providers. 2. When not in clinic, must be Flexible to accommodate client needs, including evenings and weekends.We Offer:
