• Full-time Harmony Health Home Care is seeking a dependable and compassionate professional to join our team full-time as a Client Care Coordinator.

This role provides vital support to aging adults, their families, and other individuals who need assistance with daily living. The Coordinator ensures smooth daily operations by managing client intake, scheduling, compliance with state and agency policies, and close monitoring of HHA Exchange activity. This includes daily oversight of caregiver clock-ins/outs, monthly calendar creation, authorization utilization tracking, and ongoing EVV compliance support. Conduct care consultations with clients and families, and create individualized service plans. Oversee scheduling and ongoing support to ensure quality care and client satisfaction. Prepare monthly calendars for all clients and caregivers, ensuring schedules align with authorization hours. Monitor caregiver clock-ins/outs daily via HHA Exchange, promptly addressing missed or late punches. Track utilization of authorizations to ensure service hours are delivered within approved limits. Send constant reminders to caregivers regarding EVV compliance, clock-in/out procedures, and documentation requirements. Maintain compliance reporting and escalate any recurring issues. Support HR processes including caregiver onboarding, records management, and training compliance. Maintain strong communication with clients, caregivers, families, and insurance representatives.
