Full Job Description SUMMARY:
Under the supervision of the Primary Care Provider, Registered Nurse, or Licensed Practical Nurse, the Clinic Assistant (CA) may perform participant care and support services within the clinic at
PACE SEMI.
The CA will act only within the scope of his/her authority to practice.
SPECIFIC DUTIES AND FUNCTIONS
Collaborates and communicates with the
PACE SEMI
team to promote a safe and positive work environment Greet and check-in participants for clinic appointments, including those from ancillary services Coordinates and escorts participant throughout center to designated area at appropriate scheduled times. Uses proper participant handling techniques including the use of assistant devices (ie- cane, walker, wheelchair, etc) Collaborates and communicates with clinic staff, transportation, and Day Health Center team re: cancellations, delays, and potential changes to appointments through verbal and electronic means Clerical duties such as: Answer phone, takes messages, and directs calls. Copy, fax, and file documents. Prints documents from electronic medical record Schedules appointments for participants within electronic medical record Provides appointment reminder calls Collaborates and communicates with transportation for transport of participant to and from clinic appointment Observes and reports information to primary care provider and/or nurse Obtain and document vital signs, including height and weight Assist with dressing and toileting Cleans and stocks treatment area Manages clean and dirty linen Performs other duties as assigned
KNOWLEDGE, SKILLS AND ABILITIES
High school diploma or equivalent Excellent Customer service skills Proficient in computer skills- Microsoft Office BLS Maintains annual competencies and required trainings Adheres to policies, procedures, and PACE values Prior experience or education working with geriatric population is helpful or preferred
PHYSICAL REQUIREMENTS
Physically able to walk, stand, bend, reach, and twist for prolonged periods. Able to lift 40 pounds. May be exposed to communicable disease
