Job Description

Responsibilities Clinical Outreach Liaison- IOP Mental Health We are seeking a Clinical Outreach Liaison to support access to care for our Intensive Outpatient Program (IOP) at Thousand Branches Wellness in Arden Hills, MN through clinically informed community outreach and referral engagement. This role is responsible for the execution of outreach activities with a strong clinical lens, ensuring referral partners are supported in identifying appropriate level-of-care placements. Working within established outreach priorities, the Liaison will serve as a clinical resource in the community, helping bridge gaps between providers and treatment services. This is a field-based, externally focused role and does not include responsibility for internal program operations, staffing, or clinical service delivery.
Key Responsibilities:
Execute assigned outreach plans with a focus on clinically appropriate referral development Build and maintain relationships with referral partners, including therapists, psychiatrists, hospitals, and community providers Serve as a clinical resource to the community by educating partners on levels of care, treatment modalities, and appropriate placement criteria Utilize clinical knowledge to support referral partners in determining when IOP is an appropriate level of care Conduct regular in-person outreach, meetings, and follow-ups to maintain strong community presence Collaborate with Admissions and clinical teams to support appropriate and timely referral transitions Communicate relevant clinical updates and program information to referral sources Track outreach activity and referral trends to support consistency and follow-through
