Approximately 60% of your time will be dedicated to clinical work—especially managing medication regimens for patients with chronic conditions such as diabetes—while the remaining 40% involves traditional staffing duties. Responsibilities:

Provide clinical pharmacy services with an emphasis on population health and ambulatory care Manage medication therapy for patients within target disease states, including diabetes Collaborate with healthcare providers to optimize medication regimens and improve patient outcomes Participate in staffing pharmacy operations to ensure smooth daily workflow Support program growth and development within the pharmacy department

Qualifications:

Active Pennsylvania pharmacist license Experience in ambulatory care or inpatient pharmacy settings preferred Completion of a pharmacy residency program strongly preferred Familiarity with Meditech electronic health record system is a plus Strong communication and patient care skills with a passion for clinical pharmacy
