Responsibilities Deliver direct patient care by assisting with activities of daily living such as bathing, dressing, feeding, and mobility support using equipment like Hoyer lifts. Monitor vital signs including blood pressure, pulse, respiration, and temperature; accurately document findings in electronic health records (EHR) systems such as Point Click Care. Assist with specialized care needs including Alzheimer's, dementia, memory care, behavioral health support, and working with individuals with developmental disabilities or disabilities. Maintain a safe environment by following infection control protocols, practicing aseptic techniques, and utilizing proper medical documentation procedures Experience Demonstrated caregiving experience in long-term care facilities like nursing homes or assisted living communities. Experience with medical-surgical units, post-acute care settings, or outpatient clinics is preferred. Familiarity with EMR/EHR systems such as Point Click Care or similar platforms for accurate medical documentation. Knowledge of physiology and anatomy to effectively monitor patient conditions and support complex care needs including geriatrics and pediatrics. Proven ability to work with individuals with disabilities or behavioral health challenges in diverse healthcare settings.
