Job Description

Summary/Objective:
Under the direct supervision of the Facility Manager and the Administrator, the Certified Nursing Assistant/Direct Support Professional/Direct Support Worker (CNA/DSP/DSW) performs multiple tasks for residents living at The Voyage Senior Living Herrin facility which support the ability of residents to live with maximum independence in a community setting. CNAs/DSPs/DSWs provide services within the apartments of residents, as well as, within the common areas of the facilities in accordance with a care plan developed by the Illinois Department on Aging's designated Community Care Program Case Manager on a regularly scheduled basis and respond to unscheduled and emergency needs as required to maintain a positive and safe living environment for residents.
Essential Functions/Key Responsibilities:

  • Provide residents with assistance with Activities of Daily Living (ADLs) which include, but not limited to, meal prep, eating and dining, bathing, grooming and dressing, transferring and ambulation, continence, financial and personal affairs, telephone use, laundry and housework, transportation and escort service, and self-medication assistance.
  • Assist residents with routine health directions from a physician, nurse or therapist.
  • Respond to and report resident emergencies such as falls, changes in physical/mental state, cuts, burns, and other conditions in accordance to proper procedures and regulations.
  • Respond to all natural disasters, bomb threats, fires and other environmental situations that require intervention to protect the well-being of residents and follow established emergency procedures accordingly and efficiently.
  • Reports to supervisor all concerns that may affect the well-being of residents regarding changes in health status or behavior, suspected problems with family members, general complaints, inter-resident problems, inter-staff conflicts, hazardous conditions, etc. Report any refusal of services authorized in the care plan.
  • Effectively follows written procedures and documents services provided in a clear and descriptive manner. Completes all special documentation such as incident reports and concern reports as needed.
  • Actively participates in resident social activities and provide housekeeping services in all the common areas of the facilities.
  • Performs all other duties as assigned

Physical Demands and Expectations:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
  • The employee is regularly required to talk, hear and have the ability to follow written and oral instructions and procedures
  • The position requires frequent sitting, walking and standing, occasional bending, stooping, kneeling, twisting, stretching/reaching and climbing stairs. Requires occasional lifting and carrying up to 15 pounds.
  • Must have dependable transportation and may be required to work a flexible schedule with overtime and/or shift scheduling changes to ensure that residents' needs are met
  • Must be willing to attend in-service training, seminars and other activities as assigned
  • Appropriate documentation indicating absence of infectious/communicable diseases, e.g. TB and Hepatitis may be required
  • Position is subject to a criminal background check
Education and Experience:
  • Holds a current CNA or DSP license in good standing with the state preferred.
Will need to provide a certificate of completed training if applying for the CNA or DSP position.
