TERM:
§18-5-45; extended employment terms may be established by the Jefferson County Board of Education (JCBOE)
SALARY:
Pursuant to WV Code §18A-4-2, in accordance with Jefferson County Schools Professional Salary Schedule, commensurate with experience and education level
EVALUATION
Performance in this position will be evaluated by the building principal in accordance with WV State Code §18A-2-12, WV State Board Policy 5310, and JCBOE Evaluation Policy.
JOB SUMMARY
The Communities in Schools Facilitator is responsible for the overall planning and management of the Communities in Schools (CIS) program at selected schools through the effective implementation of the CIS model providing integrated supports to students and families. The CIS Facilitator serves as an expert liaison connecting students and families with a broad variety of community- and school-based resources and services to meet the personalized needs of students. The CIS Facilitator provides case management affording access and opportunity for students to help ensure academic success and wellbeing. The position is funded through a grant sponsored by the West Virginia Department of Education.
COMMUNITIES IN SCHOOLS FACILITATOR RESPONSIBILITIES
1. Engages a school-based support team, including teachers, staff, and administrators, to nurture relationships between school and community for coordination of CIS program and services. 2. Completes referrals for students and families to school- and community-based services. 3. Recruits and trains volunteers to support the effective delivery of services. 4. Coordinates delivery of evidence-based services to address school-wide and individual students' needs through targeted and individualized support. 5. Monitors and evaluates the effectiveness of CIS program and services through systematic data collection and analysis, including an annual assessment of the program. 6. Evaluates the effectiveness of services and outcomes to meet individualized student goals. 7. Is responsible for the completion and maintenance of forms, reports and other documentation. 8. Participates actively in school-, district-, and state-level meetings to support CIS program implementation and school improvement. 9. Adheres to all school, county, and state policies and procedures. 10. Performs other tasks and responsibilities as assigned by the principal.
QUALIFICATIONS
Bachelor's degree in education, social work, or a related field required. Demonstrated experience in education, social work, or a related field required. Knowledge of community service providers and community support organizations. Knowledge of best practices in service delivery as well as child development. Ability to work cooperatively with diverse groups and individuals amidst ambiguity. Strong organizational and communication skills employing tact, patience, and courtesy. Passion and resolve for the critical role of education to improve the lives of all learners. Criminal background check conducted pursuant to WV Code §18-5-15c and/or §18A-3-10, as appropriate; meet the qualifications outlined in WV Code §18A-3-2a. WORK ENVIRONMENT
The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The noise level in the work environment is low to moderate (20-60 dB). Exceptions include gymnasiums as well as vocational/technical laboratories where the noise levels may be moderate to loud (60-90 dB).
