Position Overview
The PurposeCare CCP Program Manager is a self-starter and highly detail-oriented leader responsible for managing, training, directing, executing, and continuously improving the operations of the CCP Program. This role coordinates and oversees the CCP Program to ensure operational efficiency, compliance, and quality service delivery.
The CCP Program Manager possesses in-depth knowledge of Illinois CCP Program policies, procedures, and regulations related to local and state municipalities.
1. Personnel & Caregiver Oversight Responsible for overseeing the accuracy of personnel records
Enter mileage, show-up times, and trip fees
Manage the addition of caregivers into ClearCare (WellSky)
Manage caregiver activity, including terminations within the system as needed
Maintain personnel compliance including: TB
CPR
Licenses
Evaluations Serve as a general point of contact for caregivers
Coordinate with other agency leaders when waiver caregivers need to support private pay services 2. Start of Care (SOC) Accuracy & Compliance
Verify all SOC documentation is accurate, including: Physician information
SSN and insurance information
Proper document uploads to the patient chart
Timely addition of ADLs 3. Supervisory Visits Conduct and review all supervisor visits 4. Client Satisfaction & Documentation Ensure satisfaction surveys are completed timely and uploaded to the chart 5. Scheduling Oversight Review all schedules to ensure information is accurate at the start of services 6. Authorization Management Review authorization utilization on the 1st and 15th of each month
Verify scheduled hours are aligned appropriately against authorizations 7. Respite Coordination Partner with nursing teams to ensure respite cases have required orders for timely scheduling 8. EVV & Visit Verification Verify the number of closed visits matches what is documented in ClearCare
Send notices to caregivers regarding EVV non-compliance
Coordinate with the Regional HR Manager as needed 9. Staff Training & Leadership Train new administrative staff on CCP Program expectations
Conduct weekly and monthly meetings as necessary 10. Client & Caregiver Support Manage and resolve client and caregiver concerns 11. Billing & Operational Support Address billing issues as needed
Serve as backup support for: Case Manager
Scheduler
Staffing functions Education and Experience High School Diploma or GED required; Bachelor's Degree preferred
Minimum of five (5) years of progressive experience in business administration, preferably within healthcare, with demonstrated understanding of the healthcare environment
Minimum of five (5) years of experience managing and leading teams across multiple locations
Understanding of healthcare laws, regulations, and policies at the state, federal, and local levels
Strong understanding of Medicaid regulations preferred
Strong leadership skills with demonstrated empathy, integrity, and accountability
Intermediate to advanced proficiency in Microsoft Office Suite, particularly Excel
Exceptional interpersonal, communication, organization, and multitasking skills
Ability to prioritize effectively and allocate resources to support short- and long-term goals
Ability to work variable schedules and participate in on-call responsibilities as needed
