Job Description

A Community Habilitation Worker supports individuals with developmental, intellectual, physical, or emotional disabilities in building independent living skills and participating fully in their communities. The worker provides direct support, encouragement, and guidance tailored to each person's goals, abilities, and needs while promoting dignity, inclusion, safety, and self-confidence. Key Responsibilities Assist individuals with daily living skills such as cooking, shopping, budgeting, hygiene, and household tasks Support participation in community activities, social events, recreational programs, and appointments Encourage independence, self-advocacy, and personal growth Help individuals achieve goals outlined in their habilitation or service plans Provide companionship, emotional support, and positive behavioral reinforcement Ensure the health, safety, and well-being of individuals at all times Maintain accurate documentation of services provided, progress notes, and incidents Communicate effectively with families, supervisors, case managers, and interdisciplinary teams Assist with transportation to community activities and appointments when required Support individuals in developing communication, social, and vocational skills Qualifications High school diploma or GED required; college coursework in human services, psychology, social work, or related fields preferred Experience working with individuals with disabilities preferred Compassionate, patient, dependable, and respectful attitude Strong communication and interpersonal skills Ability to work independently and follow individualized care plans Valid driver's license and reliable transportation may be required CPR/First Aid certification preferred or willing to obtain Preferred Skills Knowledge of developmental and intellectual disabilities Crisis intervention and de-escalation techniques Experience with behavioral support strategies Cultural sensitivity and ability to work with diverse populations Strong organizational and documentation skills Work Environment Community Habilitation Workers provide services in homes and community settings, including recreational centers, stores, workplaces, and public transportation environments. Flexible schedules, including evenings and weekends, may be required.
