Job Description

Join Our Team — Community Health Worker (CHW) Cimarron Memorial Hospital is seeking a compassionate and community-focused Community Health Worker (CHW) to help connect patients with healthcare resources and support services throughout Cimarron County. This position will work closely with our hospital and Rural Health Clinic teams to: Help patients navigate healthcare and community resources Support care coordination and follow-up Assist with transportation, food, housing, and other social needs Promote healthier outcomes for our rural community Participate in tobacco cessation and wellness initiatives Ideal candidates possess the following qualifications: High school diploma or GED required Experience in healthcare, clinic, social services, community outreach, public health, or customer service preferred Strong communication and interpersonal skills Basic computer skills, including documentation and email use Ability to maintain professionalism and confidentiality Familiarity with rural communities and local resources is a plus Ability to work independently and as part of a team Compassionate, dependable, organized, and self-motivated This is an excellent opportunity to directly impact the health and wellbeing of our community while working alongside a dedicated rural healthcare team.
