Job Description

Overview Join our dedicated team as a Community Health Worker and become a vital link in promoting health, wellness, and equitable access to healthcare services within diverse communities. In this role, you will empower individuals by providing education, support, and resources tailored to their unique needs. Your enthusiasm and compassion will help foster healthier lifestyles and stronger communities through proactive engagement and personalized care. This position must be bilingual. This position is located at the Providence Medical Center. Duties Connect with community members to assess their health needs and provide culturally sensitive education on topics such as nutrition, medication management, and preventive care Assist individuals with navigating healthcare systems, including Medicare and Medicaid enrollment and understanding medical terminology Conduct outreach activities to promote public health initiatives and increase awareness of available services Utilize motivational interviewing techniques to encourage positive behavioral changes. Collect and document data related to client interactions, health outcomes, and service delivery for program evaluation Collaborate with healthcare providers, social workers, and local organizations to coordinate comprehensive interventions. Educate community members on managing chronic conditions, vaccine, prevention, cancer, geriatric, prenatal, agriculture, nutrition, obesity, food insecurity, transportation, or other health challenges through classes, trainings, presentations, outreach, resources, one-to-one coaching or case management. Requirements Prior experience or training in nursing, services, or public health is highly desirable Strong understanding of working with individuals with and diverse populations and vulnerabilities. Excellent communication skills for public speaking, teaching, and motivational interviewing Ability to collect accurate data and maintain detailed records in compliance with privacy standards Knowledge of medical terminology, Medicare and Medicaid and other health applications. Demonstrated commitment to community service with a proactive attitude toward health promotion Valid driver's license and reliable transportation preferred for outreach activities Two to four Community Workers are need to be bilingual. Pay:
$21.00 - $28.00 per hour
