COMMUNITY INTEGRATORS
!WHAT IS A COMMUNITY INTEGRATOR?
Community Integrators are direct support professionals who assist the individuals they work with to live a meaningful life in their local community. It is an ideal job for someone who enjoys a varied role and plenty of interaction with many different people throughout the day. It also gives you the opportunity to discover similar interests and incorporate your own talents, interests and hobbies into your daily work. Community Integrators provide support, companionship and fun while our individuals learn to live more independently through everyday activities they enjoy, social connections, recreational and fitness goals, educational, work or volunteer opportunities and other daily living tasks. This may include a Community Integrator assisting with personal care or grocery shopping, teaching new skills, providing emotional support or you could find yourself enjoying days out with your individuals going for walks, visiting a museum, taking day trips to local amusements and more.KEY RESPONSIBILITIES
- Assist with activities of daily living according to an individual's customized care plan
- Help to plan a weekly schedule of quality integrated activities for assigned individuals served
- Keep an accurate record of daily activities and mileage.
- Adhere to HIPAA regulations and maintain confidentiality of sensitive information
- Monitor the mental health and wellbeing of your individual and report any significant changes
- Provide emotional & physical support through active listening. Encourage clients to make good choices and achieve their personal goals
- Must maintain a valid driver's license, a reliable vehicle and proof of adequate auto insurance
OUR IDEAL CANDIDATE
- Good communication, self-directed, detail-oriented, patient, creative and compassionate
- Ability to work as part of a team in a collaborative environment
- Consistently act with honesty and integrity, while demonstrating strong commitment to quality services and the interests of individuals served
- High School Diploma/GED required; Associate degree preferred
- CPR Certification (preferred)
- Experience as a caregiver (preferred)
