Job Description

Position Summary The Community Outreach Liaison serves as the primary ambassador for Barrier Islands Free Medical Clinic , responsible for identifying, engaging, and recruiting eligible uninsured patients into care—while building strong relationships with community partners. Modeled after a college admissions recruiter, this role focuses on outreach, relationship management, and pipeline development to ensure that individuals who fall within the healthcare gap are aware of and connected to BIFMC's services. This position plays a critical role in expanding the Clinic's reach across the Tri-County area, increasing patient enrollment, and strengthening partnerships with organizations that serve vulnerable populations.
KEY RESPONSIBILITIES
Community Engagement & Recruitment Proactively identify and engage uninsured adults who may qualify for BIFMC services Develop and manage a patient recruitment pipeline, similar to an admissions funnel(prospect → inquiry → intake → active patient) Represent the Clinic at community events, health fairs, and outreach opportunities Conduct presentations to community groups, employers, and partner organizations Build awareness of BIFMC services among underserved populations Partnership Development Establish and maintain relationships with key referral partners, including: Community organizations and nonprofits Churches and faith-based groups Employers of uninsured workers (hospitality, construction, agriculture) Social service agencies Create referral pathways to increase patient access and continuity of care Collaborate with local healthcare systems and agencies to identify gaps in care Outreach Strategy & Execution Develop and implement a targeted outreach plan to reach uninsured populations across the Tri-County Identify geographic and demographic areas with the highest need Coordinate multilingual outreach efforts, particularly for Spanish-speaking populations Work closely with the communications team to align messaging and campaigns Data Tracking & Reporting Track outreach activities, referrals, and patient conversion rates Monitor key metrics: Number of new patients enrolled Referral sources Outreach event effectiveness Maintain accurate records in patient tracking systems Provide regular reports on outreach outcomes and growth trends Patient Navigation Support Collaborate with intake and clinical staff to ensure smooth onboarding of new patients Help address barriers to care (transportation, language, scheduling) Follow up with prospective patients to encourage appointment attendance Community Education Educate the public about: Preventative care Chronic disease management Available free healthcare services Promote early intervention and consistent care
Pay:
$50,000.00 - $57,000.00 per year
