CADES is a nonprofit human services organization dedicated to improving the lives of children and adults with intellectual and physical disabilities. Today, CADES operates over 30 community living arrangements, adult day programs, the George Crothers Memorial School, and early-intervention services. CADES seeks Community Support Professionals to further our mission of empowering those with special needs to achieve their highest potential.
Position Description:
The Community Support Professional (CSP) work directly with other team members to implement support plans for adults who have intellectual and/or physical disabilities. CSP's provide personal care, assistance, and guidance to enhance independence and self-worth for the individuals we serve both at our center(s) as well as through a wide variety of community experiences.
Essential Duties and Responsibilities:
Ensure service delivery that aligns with the mission, vision and regulatory requirements that positions CADES to be a role model in the delivery of adult day services to individuals with multiple disabilities and abilities. Comply with all CADES policies and procedures, state regulations as well as the Office of Developmental Program's (ODP) best practice. Implement programing within the guidelines of CADES training and regulatory standards including personal care, individual support plan (ISP)/behavior support plan (BSP) outcomes. Ensure active programming that best meets the interests and needs of the individuals served. Demonstrate effective communication skills with all stakeholders. Listen to understand, empathize and give voice to individuals who rely on assistance. Ensure written and oral communication is respectful and represent the mission and vision of the organization. Comply with all training and certification requirements to ensure the program meets all programmatic, health care, supervision and dietary needs of all individuals and ensures safety in both facility and community settings. Provide transportation, supervision, and support during community trips outings to facilitate active engagement and independence skill-building. Assist individuals in toileting, hygiene, feeding, meal preparation, and clean up as assigned to ensure personal care needs are met for the individuals you are assigned each day. Implement daily activity schedules and provide assistance as outlined in each ISP to maximize independence. Other duties as assigned.
Qualifications:
High school diploma or equivalent. Current valid drivers license with acceptable driving history. Experience or interest in working with individuals with multiple disabilities, including complex cognitive, medical and physical disabilities. Ability to instruct and assist individuals in caring for all personal needs to include feeding, toileting, and dressing. Ability to communicate clearly in writing and verbally, and the ability to read, understand written plans and instructions. Computer knowledge for email correspondence and basic data entries. This position requires the ability to perform the essential functions of the job, including the ability to lift a minimum of 20 pounds, bend, twist, and turn.
