In-Person Competitive Salary 5 Years Experience Bachelor's degree No Commission 40.00 hours per week / Day Shift / Full-Time Description The Social Worker supports students and their families through a compassionate, family-centered, and collaborative approach in an approved private school (APS) setting for students with multiple disabilities and behavioral needs. This position promotes a family-integrative model of care that honors the dignity, strengths, and unique needs of each student and their support network. Working in close partnership with an interdisciplinary team, the role oversees admissions and case management. The Social Worker also addresses family concerns using a solution-focused approach, advocates for appropriate resources, and maintains accurate and timely documentation, including progress notes and education plans. This position requires a deep commitment to equity, inclusion, and empowerment, as well as the ability to build trusting relationships with students, families, and interdisciplinary teams.
DUTIES AND RESPONSIBILITIES
Collaborate with school staff to implement and maintain a family-integrative model of care. Oversee the admissions process for students by reviewing student records and analyze information pertinent to student learning and school adjustment needs and conducting tours and interviews to determine suitability of GCMS program for the student. Conduct formal and informal observations of students as part of the admissions process. Manage a caseload of students and families and connect them to community agencies as appropriate; advocate for resources and services that meet the unique needs of each family. As a member of the transition team, advocate and support recommended educational programs for secondary. Write progress notes and maintain accurate, timely documentation in accordance with organizational and regulatory standards and participate in individualized education plan (IEP) meetings to provide information as needed to ensure an appropriate IEP. Be available and responsive to families, offering flexible meeting times and empathetic communication. Address family needs and concerns with empathy and professionalism, ensuring timely follow-up and resolution. Utilize a solution-focused approach to support families in identifying strengths, setting goals, and overcoming challenges. Provide consultation services and resources for teachers, paraeducators, and related services staff on how to support students and families. Education/Experience/Skills Requirements Bachelor's or master's in social work and valid, current licensure as an LCSW or LSW in the Commonwealth of Pennsylvania. Valid Pennsylvania Department of Education (PDE) Social Work certification or eligibility to apply for certification; PDE Special Education Teaching certification preferred. Five (5) years' experience working in family-centered or interdisciplinary care settings. Familiarity with the needs and rights of students with multiple disabilities and behavioral needs. Two (2) years' experience working with students living with multiple disabilities, including a strong understanding of the safety and communication support required for students with physical disabilities. Knowledge of teaching methods, curriculum, and education programs designed for students with learning, physical, and behavioral disabilities. Familiarity and relationships with community agencies that support individuals with intellectual disabilities. Demonstrated ability to anticipate changes and/or identify issues and take appropriate and effective actions. Excellent customer interaction, collaboration, and written and verbal communication skills. Demonstrated intermediate experience with Microsoft Office applications, including Word, Excel, Outlook, SharePoint, and PowerPoint. Experience using electronic management/electronic records systems preferred. Must be able to perform CPR, First Aid, and crisis intervention using agency-trained protocols. Must possess a current, valid driver's license in the state of residence, have a minimum of three (3) years of driving experience within the United States, and have an acceptable driving record, which will be assessed through a review of the Division of Motor Vehicles (DMV) record This position requires the ability to perform the essential functions of the job, including the ability to lift a minimum of 20 pounds, bend, twist, and turn.
