Coordinator Social Care Network (SCN)

RiseWell Community Services is seeking a compassionate Full-Time Social Care Coordinator to join our team in the Social Care Network Department. In this role, the Social Care Coordinator will be responsible for supporting the activities of Risewell's social care network operations to address unmet social determinants of health needs. The coordinator will also serve as a liaison with Risewell Community Services and the social care network lead entities. The Social Care Network Department provides services that are designed to empower both adults and children/adolescents on their journey to recovery, RiseWell offers comprehensive outpatient mental health and addiction services to individuals in need. Our mission is to provide unwavering support and guidance to individuals seeking assistance through individual and group therapy, medication-assisted treatment, and comprehensive support services.

QUALIFICATIONS

Bachelor's Degree preferred. A minimum of two years' experience completing administrative tasks. Valid and clean NY State driver's license. Fingerprinting, criminal record check, and approval from NYS Office of Mental Health.

SALARY :
