We are currently hiring a CTLP Case Assistant. As a Case Assistant, you would provide support to the Community Transition Liaison Program as well other Community Services or Clinical Services programs; help staff members and administrators achieve the agency mission and goals by relieving them of a variety of administrative and organizational details. The schedule for this full-time position is Monday- Friday, 9-5, 34 hours/ week plus unpaid one hour lunch each day. The pay range is $22.28 to $25.73 per hour. What do you need to be considered for this position? Excellent computer skills required. Ability to work independently and professionally. Valid MA driver's license and car required. High School diploma or GED with 1 year of related work experience and/or interest in working with nursing facility residents including older adults or persons with disabilities who are transitioning from an institutional to a community setting. Prolonged periods sitting at a desk and working on a computer. Must be able to lift up to 10 pounds. Travel throughout service area including the Islands. Occasional off-cape travel.
