Assessment Coordinator Angels On Duty Homecare Location:
Quad Cities/Camp Verde Area, AZ Job Type:
Part- Time (15 hours per week)
Schedule:

As an Assessment Coordinator, you'll play a vital role in ensuring our clients receive the personalized, consistent care they deserve. This position blends meaningful fieldwork with essential office coordination, allowing you to build relationships, support caregivers, and directly impact the quality of care in our community.   • Conduct initial in-home client assessments to understand individual needs and preferences.
  • Complete required 30-day and 90-day supervisory visits and reassessments to ensure ongoing quality of care.
  • Document client needs, update care plans, and communicate changes to the care team.
  • Build trusting relationships with clients and their families while delivering exceptional service and support. Office & Administrative Support
  • Maintain accurate client authorizations and documentation to support seamless care delivery.
  • Coordinate scheduling for new and existing clients, ensuring timely and reliable service.
  • Serve as a friendly and professional point of contact for clients, caregivers, and referral partners.
  • Support daily office operations and help keep systems organized and up to date. Staffing & Client Coverage
  • Help ensure every client receives care by coordinating coverage for call-outs and open shifts.
  • Reach out to caregivers to fill gaps and minimize missed visits.
  • Partner with management to quickly resolve urgent staffing needs.
  • Step in as a backup caregiver when needed, providing hands-on, non-medical care to ensure no client goes without support. Qualifications
  • High school diploma or GED required.
  • Experience in home care or a related field preferred.
  • Strong organizational, communication, and multitasking abilities.
  • Ability to work independently while managing multiple priorities.
  • Comfortable using computers and learning agency software.
  • Valid driver's license, reliable transportation, and proof of auto insurance required.
  • Willingness to travel throughout the Quad Cities and Camp Verde service areas.
  • Willingness to provide non-medical caregiving services when needed.
