Supportive Living Services Service First of Northern California - 4.0 Stockton, CA Job Details Full-time From $17 an hour 20 hours ago Qualifications Working with individuals with disabilities Working with individuals with cognitive disabilities English High school diploma or GED Driver's License Home care Time management Full Job Description Supportive Living Services (SLS) support people who want to live in their own homes. Supported living services offer an array of services that are available to anyone who desires (or whose family, advocate, or conservator desires for him or her when someone cannot express their own choice) facilitation, instruction, support, and assistance to live in the home of his or her choice.
JOB DESCRIPTION/DUTIES
: Forecast client needs and guide client goals with regard to Title 17 "Supported Living Services." Weekly visits to client homes to assess knowledge, skills, and abilities of client in functional areas of daily living (meal planning, money management, community resources, etc.). Meet clients at medical appointments. Bulk shopping. Allocate and manage IHSS hours and ensure proper administration, including direct service and billing. Provide emergency coverage for direct service staff. Participate in all meetings as needed. Other duties as assigned.
EXPERIENCE/EDUCATION
Computer and typing skills, time management, case management reporting, effective verbal and written communication skills, a self starter that takes initiative, responsible, accountability for work ethic, independent scheduling, ability to document events of the day, produce quarterly reports. Must be able to function in a fast-paced environment. Multi-tasking and multi-functioning. High School Diploma or equivalent.
HIRING CRITERIA AND QUALIFICATIONS
Experience in similar position, Valid CA Drivers' License, Reliable transportation, Proof of insurance, Ability to respect the rights and dignity of all clients Must show evidence of a negative TB test within the last year (or acquire one). Must be able to pass a fingerprint background check. Must be able to speak, read, and write English. Must be able to work with people who have disabilities
