Our agency is looking for a Service Coordinator who can make a meaningful difference in the lives of the individuals and families we serve. As a Service Coordinator, you will serve as a key point of contact for clients, families, service providers, and community partners. You will help coordinate services, communicate with stakeholders, monitor client needs, maintain accurate documentation, and ensure that services are delivered effectively and respectfully. What You'll Do Coordinate services and supports based on individual client needs and goals. Communicate regularly with clients, families, providers, and other professionals. Help clients navigate community resources and available programs. Schedule and coordinate appointments, meetings, and services. Monitor service delivery and follow up on changing needs or concerns. Maintain timely, accurate, and confidential records and documentation. Collaborate with internal staff and external providers to promote positive outcomes. Advocate for clients while maintaining professionalism, confidentiality, and respect. Respond to challenges with sound judgment, empathy, and problem-solving skills. Qualifications may include:

Experience in customer service, scheduling, healthcare, social services, or a related field. Strong written and verbal communication skills. Excellent organization and time-management abilities. Ability to work independently while collaborating effectively with a team. Strong attention to detail and documentation skills. Ability to maintain confidentiality and professional boundaries. $25.00 - $27.00 per hour
