Designated Coordinator (HCBS) Northeastern Home Group - 5.0 Edina, MN Job Details Full-time $24 - $28 an hour 10 hours ago Qualifications Google Workspace Interpersonal skills Driver's License Attention to detail Organizational skills Full Job Description Position Summary The Designated Coordinator is responsible for overseeing client services, staffing coordination, compliance monitoring, communication with case managers and families, and ensuring services are delivered according to Minnesota 245D requirements. The ideal candidate is highly responsive, detail-oriented, and able to manage multiple priorities while maintaining excellent communication and follow-through. Key ResponsibilitiesClient Coordination Serve as primary point of contact for assigned clients. Maintain regular communication with clients, guardians, families, and case managers. Conduct client visits and monitor service quality. Ensure person-centered plans are implemented and followed. Staffing & Recruitment Monitor staffing needs daily. Recruit, screen, and interview candidates. Coordinate staff onboarding and training. Fill open shifts and staffing gaps quickly. Maintain an active pipeline of qualified candidates. Compliance & Documentation Monitor annual reviews and service plan requirements. Ensure documentation is completed accurately and on time. Track staff training requirements and expirations. Assist with audits and compliance reviews. Maintain accurate client and staff records. Communication & Follow-Up Respond promptly to emails, phone calls, and team communications. Provide regular updates to leadership. Escalate concerns before they become problems. Follow through on assigned tasks without repeated reminders. What Success Looks Like The successful candidate will: Keep clients staffed and supported. Follow up consistently. Respond to communication promptly. Take initiative without being asked. Stay organized and meet deadlines. Build positive relationships with clients, families, staff, and case managers. Help Northeastern Home Group grow through strong service and accountability. Qualifications Experience in HCBS, healthcare, social services, or human services preferred. Knowledge of Minnesota 245D preferred. Strong communication and organizational skills. Ability to manage multiple priorities. Experience recruiting and interviewing preferred. Valid driver's license and reliable transportation. Proficient with Microsoft Office, Google Workspace, and online systems.
Pay:
$24.00 - $28.00 per hour
