PRIMARY RESPONSIBILITIES
Job Summary Anytime Home Care is seeking a highly organized and detail-oriented individual to serve in a dual role as both an Administrative Assistant and a Designated Coordinator under Minnesota's 245D Home and Community-Based Services. This person will be fully responsible for the day-to-day administrative operations of the office while ensuring all client and employee files, forms, and documentation remain fully compliant with 245D licensing standards. This is a hands-on position ideal for someone who thrives in a structured environment and understands the importance of timely documentation, regulatory compliance, and responsive communication.Key Responsibilities Administrative Responsibilities:
Answer incoming phone calls and professionally respond to emails Manage the organization of both client and employee files , ensuring they are up to date and kept separate per DHS requirements Organize and maintain both physical and digital filing systems Schedule and coordinate meetings, staff training, and deadlines Monitor office supplies and place orders as needed Track and assist with onboarding new employees Support payroll preparation and timesheet organization Assist in preparing internal reports and data summaries as needed Designated Coordinator Responsibilities (per MN 245D requirements): Develop, complete, and maintain IAPP forms , CSSP Addendums , and any other required support plans for each client Review and ensure all client-related documentation is complete, accurate, signed, and compliant Monitor service delivery to ensure it aligns with each individual's plan Coordinate and document team meetings with clients, guardians, case managers, and staff Regularly update and monitor service outcomes and progress documentation Communicate with Direct Support Professionals (DSPs) and provide training or updates as needed Conduct quality assurance reviews of each client's file and staff file regularly Ensure all files are audit-ready at all times Notify leadership of compliance issues or documentation gaps including, but not limited to: Developing and/or writing and implementation of each client's person-centered support plans, (CSSP) Coordinated Service and Support Plan, (SMA) Self-Management Assessment, and (IAPP) Individual Abuse Prevention Plan. Maintaining all signatures as required by 245-D licensing rules and regulations. Maintaining, updating all files as required by 245-D licensing rules and regulations. Compliance with 245-D licensing rules and regulations. Writing of quarterly and annual reports as needed or directed by the person served and/or team to fulfill person-centered plan. Able to problem solve as needed and/or be creative to problem. Conduct client intake, admission and assessments. Attend client Support Team meetings as needed Develop and implement client CSSP-A's Review client documents so that they meet licensing guidelines (Annual Outcomes and Progress Reviews and other related reports), keeping a working knowledge of regulatory requirements Maintain accurate records and reports Serve as a communication liaison with guardians and other service providers Review incident and emergency reports, identification of incident patterns and implementation of correctives actions Serves as a client advocate Education and experience requirement. The Designated Coordinator and must minimally meet one of the following: 1. A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older 2. An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; 3. A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; 4. A minimum of 50 hours of education and training related to human services and disabilities; and; four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meetsJob Type:
