Minneapolis, MN Job Details Full-time From $24 an hour 4 hours ago Qualifications Microsoft Office Driver's License Attention to detail Human Services Care plans Full Job Description Position Summary The Designated Coordinator (DC) is responsible for overseeing and coordinating services for individuals receiving support under Minnesota 245D Home and Community-Based Services standards. The Designated Coordinator ensures services are person-centered, compliant with state regulations, and aligned with each individual's assessed needs and goals. This position serves as a liaison between clients, families, staff, case managers, and other providers to ensure quality service delivery and ongoing support. Essential Duties and Responsibilities Coordinate and oversee services provided to individuals receiving 245D services. Develop, review, and maintain required service documentation, including Coordinated Service and Support Plans (CSSP), addendums, and individualized support plans. Ensure services are implemented according to client goals, preferences, and assessed needs. Conduct client intakes, assessments, orientations, and service planning meetings. Monitor service delivery and ensure compliance with Minnesota 245D licensing standards and company policies. Maintain accurate and timely documentation, case notes, incident reports, and progress updates. Provide ongoing communication and coordination with guardians, case managers, families, and interdisciplinary teams. Supervise and support direct support staff to ensure quality care and professionalism. Assist with staff training, coaching, scheduling support, and performance monitoring as assigned. Respond appropriately to incidents, emergencies, and client concerns. Ensure health, safety, and rights protections for all individuals served. Participate in audits, quality assurance activities, and compliance reviews. Maintain confidentiality and professionalism at all times. Qualifications Bachelor's degree in Human Services, Social Work, Psychology, Education, Healthcare, or a related field preferred. 5-7 years of experience with individuals with disabilities, mental health needs, autism, or behavioral support needs required if no Bachleor's degree. Knowledge of Minnesota 245D regulations and person-centered practices preferred. Strong organizational, communication, and problem-solving skills. Ability to manage multiple cases and deadlines effectively. Proficiency in documentation and electronic record systems. Valid driver's license and reliable transportation preferred. Must pass required background studies and trainings. Preferred Skills Leadership and team coordination abilities. Knowledge of HCBS waiver services and community resources. Ability to build positive relationships with clients, families, and staff. Strong attention to detail and compliance standards. Physical Requirements Ability to travel to client homes, community locations, and meetings as needed. Ability to sit, stand, bend, and move throughout the workday. Ability to respond to emergencies and crisis situations appropriately. Work Environment Services are provided in office, home, and community-based settings. The Designated Coordinator may work directly with clients, staff, guardians, and community professionals in a fast-paced and supportive environment.
